Deep Calf Muscle Strain
You’ve strained, or partly torn, the soleus, the deeper calf muscle that sits underneath the main one. It usually creeps in as a deep, cramp-like ache rather than a sudden pop, often while running. This muscle does most of its work when your knee is bent, which is exactly how we’ll pinpoint it and how we’ll train it back.
The soleus does most of the work in steady running and standing, so it tends to get overloaded by a jump in mileage rather than one explosive moment. The fix is graded loading biased toward the BENT-KNEE position: settle it, rebuild seated calf raises, add slow-lowering bent-knee heel drops, then return to running.
How it typically shows up
A deep, often cramp-like ache in the lower calf that usually builds with running mileage rather than one sudden pop. Pain reproduced by loaded calf work with the knee bent about 90 degrees, plus deeper or lower tenderness, points toward the soleus rather than the gastrocnemius.
How long recovery takes
Soleus strains recover well over a few weeks by gradually building it back up, with the work done knee-bent: settle it down and start gently moving, rebuild seated (knee-bent) calf raises, add slow heel drops and a knee-bent calf stretch, then work back up to running. You move to each stage once you’re pain-free with no extra swelling.
The phased recovery approach
Phase 1 · 1–2 weeks
Calm
Calm the deep calf down first, gentle movement only while the strain settles.
What you get back: walking without the deep ache.
- Ankle Pumps — 2–3 sets × 10–15 reps · Point your foot down away from you.
- Ankle Circles — 2–3 sets × 10–15 reps · Sit or lie down with your leg supported.
- Bent-Knee Calf Push (seated, soleus) — 2–3 sets × 10–20s hold · Sit with your knee bent to about 90 degrees.
- Isometric Calf Push (seated) — 2–3 sets × 10–20s hold · Sit with your foot flat on the floor or against a wall.
Phase 2 · 2–5 weeks
Rebuild
Rebuild the deep calf’s strength with bent-knee work until it can take load again.
What you get back: pushing off with full strength.
- Ankle Circles — 1–2 sets × 8–10 reps · Sit or lie down with your leg supported.
- Ankle Pumps — 1–2 sets × 8–10 reps · Point your foot down away from you.
- Seated Heel Raise (bent-knee, soleus-bias) — 3 sets × 12–15 reps · Sit with your knee bent, adding weight on your thigh as you progress.
- Single-Leg Heel Raise — 3 sets × 12–15 reps · Rest a fingertip on a wall for balance.
Phase 3 · 2–6 weeks
Back to running
Build the deep calf’s spring and running load back, it does most of the work when you run.
What you get back: running again.

- Loaded Calf Raise — 3 sets × 15–20 reps · Hold a weight or use a calf machine, starting with your heels at floor level.
- Backpack Seated Calf Raise (bent knee) — 3 sets × 15–20 reps · Sit with the working knee bent to about 90 degrees and place a loaded backpack securely across that thigh.

What matters while you recover
Train the soleus with a bent knee
The soleus doesn’t cross your knee, so it works when the knee is bent. That’s why your calf work is bent-knee, loading it where it actually does the job.
How much pain is okay
Early on stop at pain. Later, up to 4/10 that settles is fine, not the 5/10 tendon rule. Before sport get full strength, range, and a brisk walk with no pain or swelling.
Common questions
- How is this different from a normal calf strain?
- A ‘normal’ calf strain is usually the gastrocnemius, the big muscle you feel when your knee is straight, often torn by a sudden push-off (‘tennis leg’). The soleus is the deeper muscle underneath, worked with the knee bent, and it tends to creep in with running rather than pop. Same muscle-tear idea. We just bias your exercises to the bent-knee position to load the soleus.
- Why does the knee-bent test matter?
- The gastrocnemius crosses the knee, so when you bend your knee it goes slack and stops sharing the load, leaving the soleus to do the work. So if your calf pain shows up with the knee BENT (and not so much knee-straight), it points to the soleus. That’s also why your strengthening is done knee-bent.
- How much pain is okay during the exercises?
- Early on, stop at the point of pain and don’t push into it while it’s still healing. As you progress, a little discomfort up to about 4/10 that settles is fine. This is a muscle tear, so it’s NOT the more permissive ‘some pain is fine’ tendon rule.
